Standard 8VAC20-780-160-C
At least every two years from the date of the initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening. Two of the four staff records reviewed had expired tuberculosis screenings/tests.
Plan of Correction: The center?s plan of correction: The two staff members were able to make appointments to obtain a follow-up tuberculosis screening, and the appointments were complete. The results of the negative tests will be kept in the staff?s file

Repeat Violation:
Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition. In the children?s bathroom, there is a cracked toilet lid and a screw protruding out of the side of the commode. The area rug carpet in the 3-year-old classroom has corners of the rug folding in on each edge posing as a tripping hazard to the children in care. The indoor basketball hoop did not have a strong base and would easily tip over posing as an injury hazard.
Plan of Correction: The center?s plan of correction: The children?s bathroom will have a new toilet lid as a new one was ordered and will be replaced. The screw to the side of the commode will have a cover. On the day of the inspection, the area rug carpet and the basketball hoop was removed from the center and will no longer be used.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. Disinfectant sprays with the warning label, ?Keep out of Reach of Children,? were observed in the preschool and infant room that were both unlocked.
Plan of Correction: The center?s plan of correction: The center?s Director had a meeting with the staff to explain the importance of keeping supplies locked up. They substances were locked up during the time of the inspection.
