Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A subsidy health and safety (SHSI) inspection was initiated and concluded on 11/15/2021. There were 2 children present with 1 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of records and interview, the provider's annual training did not include the
department's annual health and safety update course.
Evidence:
The provider's annual training documented in her record did not include record of having completed the annual Health and Safety update training.
The provider confirmed that she had taken the Virginia Preservice Training when she became a subsidy vendor in September 2020 but was not aware that an annual health and safety update to this training was required.
Plan of Correction: The provider stated that she would complete the annual Health and Safety update training this week and submit a copy of the certificate of completion to the licensing inspector.
