Inspector Notes

A renewal inspection was initiated and concluded on 4/15/2022. There was one -three-year-old child present, with the licensee supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 provider / household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of records and interview, the provider did not ensure that every caregiver has documentation of a subsequent Tuberculosis screening within 2 years of their last Tuberculosis screening.
Evidence:
The most recent Tuberculosis screening for caregiver #1 was dated 3/16/2020 and was more than 2 years old.
The provider stated that she had not yet obtained an updated Tuberculosis screening and had not realized it was expired.
Plan of Correction: The provider obtained a new Tuberculosis screening following the inspection and forwarded the results to the inspector on 4/18/2022.
