Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 10-8-2024 and completed on 10-9-2024. The on-site inspection began at 11:55am and ended at 1:55pm. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 caregivers; the point total was 11. The inspector reviewed 3 children?s records and 1 provider record and 1 household member record on-site. This inspection included document review, tour of the family day home, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10-17-2024. A POC submitted after this date will not appear on the public website.
Standard 22.1-289.058
The family day home that was built before 2015 and serves preschool-age children shall be equipped with at least one carbon monoxide detector. The home did not have a carbon monoxide detector on site at the time of the inspection.
Plan of Correction: Second carbon monoxide detector was purchased and will be installed 10/12/2024.
Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There was not a physical exam record available for Child #2, who has been in care for over 60 days.
Plan of Correction: Mom dropped form off for completion by PCP.
Standard 8VAC20-800-250-A
Hanging items including appliance cords, shall be out of reach of children under five years of age. A child 17 months old was observed playing with a television cord that was plugged in and hanging from the television mounted on the wall.
Plan of Correction: Purchased an attached cord protectors and secured loose TV and apple TV cord
Standard 8VAC20-800-280
Poisonous materials shall be stored in an area inaccessible to children. Poisonous cleaning chemicals were accessible to children, many chemical containers were observed on a shelf in a closet and another chemical was on the bathroom counter.
Plan of Correction: Closet will remain locked with door knob lock.

Providers must keep a written record of children in attendance each day. There were no attendance records as of 11:55am on the date of inspection. The children had been in care for more than one hour.
Plan of Correction: Attendance completed daily.

Cribs shall be provided for children from birth through 12 months. A child 8 months old does not have a crib and provider indicated they sleep on a cot.

Standard 8VAC20-800-530-A
Cots when being used for napping by children other than infants shall have linens consisting of a top cover and a bottom cover or a one-piece covering that is open on three edges. There were no linens available during naptime for the cots.
Plan of Correction: Linens purchased and applied to cot.
Standard 8VAC20-800-680-A
Caregivers shall wash their hands with liquid soap and warm running water after diapering a child or assisting a child with toileting. Provider did not wash their hands in between 3 separate diaper changes and after.
Plan of Correction: Handwashing will occur after each diaper change for child.
Standard 8VAC20-800-680-B
Caregivers shall ensure that children's hands are washed with liquid soap and warm running water after toileting or diapering. Three children had their diapers changed and the provider did not ensure they washed their hands after diapering.
Plan of Correction: Diapering will occur in bathroom and hand washing will occur after each diaper change.
Standard 8VAC20-800-690-D
The diapering surface shall be separate from areas used for children?s activities and the diaper pad shall be cleaned and sanitized after each use. During diaper changing the diaper pad was placed on furniture (an ottoman), that the children use. The provider changed 3 children?s diapers and used a baby wipe to wipe down the diaper pad in between each use. The diaper pad was not cleaned and sanitized as required.
Plan of Correction: Diapering will occur in bathroom with foot pedal and lid trash can. Handwashing in between changes will occur. Diaper pad will be sanitized and washed in between.
Standard 8VAC20-800-690-E
Soiled disposable diapers and wipes shall be disposed of in a leak-proof or plastic lined storage system that is either foot operated or used in such a way that neither the caregivers hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal. Soiled diapers were not disposed of in a foot operated tstorage system.
Plan of Correction: Diapering will occur in the bathroom with foot pedal trash can with lid.
Standard 8VAC20-800-880
Family day homes shall follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). The USDA requires 5 components to be served for lunch to include fluid milk. Children in care were served pizza rolls, french fries and juice for lunch and they were not served milk or a fruit as required.
Plan of Correction: Lunch will consist of five components each day with milk and fruit.

When meals or snacks are provided by the family day home, the menu for the current one-week period shall be dated. The menu posted did not include a month or date.
Plan of Correction: Menus will be dated with the month and the week.
