Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 7-22-2025 and completed on 7-28-2025. The on-site inspection began at 1:20pm and ended at 3:00pm. The inspector reviewed compliance in the areas listed above.
There were 7 children present and 1caregivers; the point total was 15. The inspector reviewed 5 children?s records and 1 caregiver/ provider record and 1 household members records on-site.

This inspection included document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8-5-2025. A POC submitted after this date will not appear on the public website.

A landline telephone, excluding a cordless or cell phone, shall be available, operable, and accessible during the family day home?s hours of operation. An operable landline telephone is one that does not require electricity to operate. There was not a landline telephone available.

Repeat Violation
Cribs shall be provided for children from birth through 12 months of age and for children over 12 months of age who are not developmentally ready to sleep on a cot, rest mat, or bed. Child #1 under 12 months of age was observed sleeping in the bouncy seat and there was not a crib set up and ready for use for Child #1.

A working battery-operated flashlight, a working portable battery-operated weather band radio, and extra batteries shall be kept in a designated area and be available to caregivers at all times. These items were not available to view in the family day home during inspection
