Inspector Notes

An unannounced monitoring inspection was conducted today, July 17, 2023, as a follow-up to the issuance of a provisional license. The inspector was on site from approximately 1:00 pm until 1:30 pm. There were three children present with one caregiver supervising for a value of seven points. Children were observed napping. The inspector reviewed compliance in the areas of previously cited: administration, caregiver training, environment and emergencies.
One child record was reviewed and training records for two caregivers were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of one children?s records, the provider did not obtain documentation of the
child?s physical prior to the child?s attendance or within 30 days after the first day of attendance.

Evidence:

The record of Child #1, enrolled 09/13/2022, did not have documentation of a physical exceeding 30 days from the first day of attendance.
Plan of Correction: Per the provider: I will call the parent and have her provide a copy. I thought when she gave me the immunizations, that the physical was included.
