Based on the review of 10 staff records, the center did not obtained a completed sworn statement or affirmation before two staff members beginning employment.

Evidence:
The records for Staff #4 (Hire date 04/01/2022) contained a signed sworn statement dated 03/23/2022. The documented sworn statement was signed by Staff #4. The questions including: Have you lived outside of Virginia in the past five years?, Have you ever been convicted of or are you the subject of pending charges of any crime within the Commonwealth of Virginia?, Have you ever been convicted of or are you the subject of pending charges of any crime outside the Commonwealth of Virginia?, Have you ever been the subject of a founded complaint of child abuse or neglect within the Commonwealth of Virginia?, Have you ever been the subject of a founded complaint of child abuse or neglect outside the Commonwealth of Virginia? were not completed and left blank.
The records for Staff #5 (Hire date 06/02/2022) contained a signed sworn statement dated 05/06/2022. The documented sworn statement was signed by Staff #5. The questions including: Have you lived outside of Virginia in the past five years?, Have you ever been convicted of or are you the subject of pending charges of any crime within the Commonwealth of Virginia?, Have you ever been convicted of or are you the subject of pending charges of any crime outside the Commonwealth of Virginia?, Have you ever been the subject of a founded complaint of child abuse or neglect within the Commonwealth of Virginia?, Have you ever been the subject of a founded complaint of child abuse or neglect outside the Commonwealth of Virginia? were not completed and left blank.

Based on the review of 10 staff records, the center did not obtain documentation of a negative tuberculosis screening for two staff members within the required time frame.

Evidence:
The records for Staff #7 and Staff #8 did not contain documentation of a negative tuberculosis screening. Tuberculosis screenings are required to obtained within 30 calendar days prior to beginning employment and be signed by a physician, physician's designee, or an official of the local health department.

Based on review of 10 children's records, the center did not ensure seven children's records contained the required information.

Evidence:
1. The records for Child #4, Child #5 and Child #6 did not contain name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
2. As of 06/28/2022, members of management did not submit records for Child #7, Child #8, Child #9 and Child #10.

Based on the review of 10 children's records and documentation review, the center did not obtain a written care plan for one child diagnosed food allergy. Written care plan is required to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the vent of the suspected or confirmed allergic reaction.

Evidence:
The record for Child #5 indicated the child has a food allergy. The submitted record did not contain a written care plan. During the 06/09/2022 onsite inspection, the allergy list located in the center confirmed Child #5 had a documented food allergy.

Based on review of 10 staff records, the center did obtain the required information for four staff members.

Evidence:
1. The records for Staff #4, Staff #5, Staff #7 and Staff #8 did not contain documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position, and orientation and training.
