Inspector Notes
The licensing inspector conducted an unannounced monitoring inspection in response to a self-report that was received by the Office of Child Care Health and Safety on June 12, 2023, regarding the area of physical plant and injury reporting. The investigation was initiated on June 12, 2023 and concluded on June 12, 2023. On June 12, 2023, the inspector was on site at the center from approximately 12:07pm to 12:20pm. Interviews with center staff were completed, observations were made and documentation was reviewed.
The self-report is determined to be unfounded. As a result there were no violations issued.
