During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 05/29/2025 and completed on 06/02/2025. The on-site inspection began at 3:35pm and ended at 5:20pm. The inspector reviewed compliance in the areas listed above. There were 26 children present and four staff. The inspector reviewed five children?s records on-site and five staff records electronically on 06/02/2025.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations

Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #2, who had been enrolled for nine months.
Plan of Correction: We will remind the parents again to submit a physical exam.
Standard 8VAC20-780-280-B
Hazardous substances must be in a locked place that prevents access by children. An unlocked cabinet in the snack kitchen contained cleaning products. Children had access to the snack kitchen area during the inspection.
Plan of Correction: Unlocked cleaning chemicals were relocated during the inspection.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1, enrolled for six months did not contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
Plan of Correction: We will ensure to obtain the missing information and add it to the record.
