Inspector Notes

An unannounced monitoring inspection was conducted on 01/25/2024. There were 27 children, ages 2 years-4 years, and 8 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed during free choice play, participating in circle time and learning activities, and having a morning snack. A total of 5 children?s records, 5 staff records, and 6 medications were reviewed. The inspector arrived for the inspection at 8:45 am and departed at 11:00 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review and staff interviews, the center failed to obtain a copy of the child abuse and neglect registry check from any state in which the individual has resided in the preceding five years.

Evidence: The record for Staff 5 contained evidence that Staff 5 had resided outside of the state of Virginia within the preceding five years. Staff 5 verified that they had resided outside of the state of Virginia withing the preceding five years. The record for Staff 5 contained a documented hire date of 12/13/2023. There was not a child abuse and neglect registry check from all of the states in which the individual had resided in the preceding five years available for review during the 01/25/2024 inspection. Staff 5 was observed working during the 01/25/2024 inspection.

Based on record review, the center failed to obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence: The record for Child 5 contained a documented start date of 03/27/2023. The most recent immunization record in the record for Child 5 was dated 03/24/2023; additional immunizations have not been obtained every 6 months from 03/27/2023 until the time that the child turned 2 years old.
Plan of Correction: Will request the information from the parent.
