Inspector Notes

An unannounced monitoring inspection was conducted on 07/01/2024. There were 43 children, ages 18 months-12 years, and 9 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed participating in self-guided activities, teacher lead activities, participating in music and movement, in outdoor large motor play, during lunch and going down for nap. A total of 5 children?s records, 5 staff records, and 5 medications were reviewed. The inspector arrived for the inspection at 9:55 am and departed at 12:00 pm

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review, the center failed to obtain documentation of a negative tuberculosis screening at the time of employment, that shall have been completed within the last 30 calendar days of the date of employment.

Evidence: The record for Staff 3 contained a documented hire date of 06/28/2024; the tuberculosis screening was dated 03/27/2024.
Plan of Correction: TB screenings more than 30 days old will not be accepted.
