An unannounced, on-site renewal inspection was initiated and completed on 07/14/2025. The on-site inspection began at 9:30 am and ended at 11:10 am. The inspector reviewed compliance in the areas listed above. There were 41 children and 9 staff. The inspector reviewed 5 children?s records, 5 staff records, and 1 applicant record on-site.

The inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The sworn statement for Staff 5 was dated 2 days after their first date of employment.
