Inspector Notes

This inspection was conducted by licensing staff both on-site and using an alternate remote protocol, including telephone contacts, documents review, interviews and a tour of the program.

A monitoring inspection was initiated on 3/15/22 and concluded on 3/31/22. The center director was contacted by telephone to initiate the inspection and an on-site inspection was conducted on 3/31/22 from 10:15am until 11:35am. At the time of the on-site inspection, there were 38 children in care with 7 staff present. A sample of 4 children's records and 4 staff records were reviewed. Children were observed engaging in morning learning activities and free play. Restroom and handwashing procedures were also observed. First aid and emergency supplies, documentation of emergency practice drills, transportation, children's injury reports, the emergency preparedness plan and required center postings were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation and interview, the center did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
1. There is a hole approximately 3 inches in diameter with exposed drywall in the wall of the bathroom in the pre-k classroom and is within reach of the children in care.
2. The center director confirmed that the wall in the pre-k bathroom has a hole in it that is within reach of children.
Plan of Correction: The center responded with the following: The hole in the pre-k bathroom will be repaired as soon as possible. We will look into placing a door stop to prevent future holes from occurring.
