Inspector Notes

A monitoring inspection was conducted on 3/1/2022. There were 9 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 children's records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-240-A
Based upon review of staff records and staff interview, the facility has not ensured that all staff complete the Virginia department of Education sponsored orientation course within 90 calendar days of employment.
Evidence:
1. According to staff 1, staff 3 was hired on 8/1/2021. There was no documentation showing that staff 3 completed the required orientation course.
2. According to staff 1, staff 4 was hired on 9/4/2021. There was no documentation showing that staff 4 completed the required orientation course.
3. Staff 1 verified that there was no documentation showing completion of the required orientation course for staff 3 or staff 4.

Standard 8VAC20-780-330-B
Based upon observation, the facility has not ensured that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials on pages 6-7 of the National Program for Playground Safety's "Selecting Playground Surface Materials under equipment with moving parts to create a fall zone free of hazardous obstacles.
Evidence:
There is a saucer swing in the grassy area on the playground. There is no resilient surfacing under or in the fall zone of the swing.
