Inspector Notes
A renewal inspection was conducted on 05/02/2023. There were 7 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Based upon review of staff records and staff interview, the facility has not ensured that the results of a search of the child abuse and neglect registry have been obtained from any state in which an applicant has resided in the past five years.
Evidence:
1. The record provided for staff 2 did not include the results of a central registry search from the state of Florida, where, according to the Sworn Disclosure Statement provided, the employee lived within the past five years.
