Inspector Notes

A monitoring inspection was conducted on 10/11/2022. There were 19 children present and 3 staff. A tour of the facility was conducted, in addition to a record review and interviews with staff. The Inspector reviewed 2 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Based on observation the center did not ensure all areas and equipment of the center, inside and outside were kept in a safe condition.
Evidence:
There was peeling paint along the wooden walkway leading into the center. The wood was also splintered and rough to touch.
Plan of Correction: The following corrective action was provided by the director during the exit interview:
The Director has already addressed this with the landlord of the building. The Director will contact the landlord to discuss a plan to have this corrected.
Standard 8VAC20-780-450-A
Based on observation and interview, the center did not ensure all requirements were met during the designated rest period.
Evidence:
During nap a child was observed sleeping on a cot with no bottom cover. The director stated they did not have any extra bottom covers to provide the child.
Plan of Correction: The following corrective action was provided by the director during the exit interview:
Several children forgot their linens and the center had distributed all they had. The center will ensure there is enough linen for all children in the event they forget theirs at home.

Based on a record review the center did not ensure all required information was kept for each staff person.
Evidence:
Staff #1, hired 09/06/22, as a program leader, did not have documentation in the the record that demonstrated the individual possessed the education, and/or certification required by the job position.
Plan of Correction: The following corrective action was provided by the director during the exit interview:
The director will request a copy of staff #1's transcript to keep in the record.
