Inspector Notes

An unannounced, on-site monitoring inspection was completed on 09/20/2024, as a part of licensure period. The on-site inspection began at 12:00pm and ended at 2:00pm. The inspector reviewed compliance in the areas listed above. There were 17 children present and 4 staff. The inspector reviewed 2 children?s records and 3 staff records. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
The center is required to maintain areas and equipment of the center, inside and outside on a safe condition.

1. In the children's bathroom potential suffocation hazards, an open box of plastic gloves and an open roll of plastic bags, were on top of the toilet tank lid and in reach of children.
2. On the playground there was a rusted tricycle with missing handles.
3. Areas of the front entrance walkway and ramp had peeling paint.
Plan of Correction: the plastic gloves and bags were moved to an area not in reach of children, the tricycle was removed rom the playground. The owner will contact the landlord of the building to correct the peeling paint.

(REPEAT VIOLATION)
Where playground equipment is provided, resilient surfacing shall comply with
minimum safety standards when tested in accordance with the procedures
described in the American Society for Testing and Materials standard F1292-99.

The spring rocker had no resilient surfacing under the piece of equipment.
Plan of Correction: The rocker will be removed from the playground and replaced with equipment that does not required resilient surfacing. The rocker will be blocked off with tape until it can be removed.

The ground supports of outdoor play equipment are required to be covered with materials that protect children from injury.

The ground supports of the spring rocker on the playground was not covered with materials to protect children from injury.
