Inspector Notes

An unannounced renewal inspection was completed on 10/07/2025 as a part of the licensure period. The on-site inspection began at 11:00am and ended at 12:30pm.The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. There were 17 children present and 6 staff. The inspector reviewed 5 children?s records and 2 staff records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-150-B
Immunizations reports shall include the date of the physical examination and dates
immunizations were received and shall be signed by a physician, his designee, or an
official of a local health department. The immunization report for child #4 was not signed by a physician, his designee, or an official of a local health department.
Plan of Correction: The director will contact the parent and have them resubmit the immunization record after it has been signed by the child's physician.
Standard 8VAC20-780-210-A-4
Staff who are program leaders are required to meet at least 1 of 4 criteria. The center qualified Staff #1 as having 6 months experience and 24 hours of training. however Staff #1 only had 20 hours of documented training.
Plan of Correction: Staff #1 has almost completed all of the 24 hours of training. She should be complete by next week.

The center is required to have all first aid items in their kit. The center did not have 1 of the 2 required triangular bandages in the first aid kit.
Plan of Correction: The center had 1 triangular bandages and will obtain an additional one for the first aid kit.
