Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in-person tour of the family day home. A subsidy inspection was initiated on 2/22/22 and concluded on 2/24/22. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of the emergency drill log and interview, it was determined that the facility did not ensure that a lockdown drill is completed annually.

Evidence:
1. The emergency drill log did not have written documentation to demonstrate that a lockdown drill was completed during 2021.
2. Caregiver #1 confirmed that there was no lockdown drill completed during the 2021.
Plan of Correction: The provider responded: We make sure that lockdown drill is completed annually.
