Inspector Notes
A monitoring inspection was conducted on 06/10/2024. A tour of the facility was conducted, in addition to a record review and interviews with staff. The Inspector reviewed children?s and staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Caregiver #1 and #2 did not have the results of fingerprinting, 5 years from the date of the last one. Both checks were dated 12/12/2018.
Evidence:
1. Caregiver #1 did not have the results of a central registry check, 5 years from the date of the last one. The last central registry check for Caregiver #1 was dated 08/23/2018.
2. Caregiver #1 and #2 did not have a signed sworn statement of affirmation, 5 years from the date of the last one. Both sworn statements were dated 12/28/2018.
Evidence:
The licensee did not have documentation of annual training in 2023. The licensee confirmed she did not have any training that year.
Evidence:
The laundry room door was open and accessible to the main daycare area. There were various cleaning agents on the washing machine, accessible to the children in care.
