Inspector Notes

A monitoring inspection was conducted on 06/10/2024. A tour of the facility was conducted, in addition to a record review and interviews with staff. The Inspector reviewed children?s and staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-B-2
Based on a record review the licensee did not ensure all caregivers and household members had the required background checks.
Evidence:
Caregiver #1 and #2 did not have the results of fingerprinting, 5 years from the date of the last one. Both checks were dated 12/12/2018.
Plan of Correction: The licensee will schedule an appointment for fingerprinting this week. Results will be sent to the inspector once received.
Standard 8VAC20-770-40-D-4
Based on a record review the licensee did not ensure all caregivers and household members had the required background checks.
Evidence:
1. Caregiver #1 did not have the results of a central registry check, 5 years from the date of the last one. The last central registry check for Caregiver #1 was dated 08/23/2018.

2. Caregiver #1 and #2 did not have a signed sworn statement of affirmation, 5 years from the date of the last one. Both sworn statements were dated 12/28/2018.
Plan of Correction: All background checks will be redone for all caregivers and household members.
Standard 8VAC20-800-210-B
Based on a record review the licensee did not ensure they received the required annual training.
Evidence:
The licensee did not have documentation of annual training in 2023. The licensee confirmed she did not have any training that year.
Plan of Correction: The licensee has already completed training for 2024 in an effort to make-up for missed training and to meet the 2024 requirements.
Standard 8VAC20-800-280
Based on observation the licensee did not ensure potentially poisonous substances were inaccessible to children.
Evidence:
The laundry room door was open and accessible to the main daycare area. There were various cleaning agents on the washing machine, accessible to the children in care.
Plan of Correction: The laundry room door was closed and locked during the inspection.
