Inspector Notes

An unannounced monitoring inspection was completed on 12/04/2024, as a part of the licensure period. The on-site inspection began at 5:00pm and ended at 6:30pm.The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. There were 4 children present and 2 caregivers; the point total was 4. The inspector reviewed 4 children?s records and 2 caregiver/household member(s) records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-A
Each Caregiver and adult household member of the family day home are required to provide a signed sworn statement or affirmation disclosing whether he has been the subject of a founded complaint of child abuse or neglect within or outside the Commonwealth every five years.

Caregiver #1 and Caregiver #2 did not have an updated sworn statement of affirmation. Both were dated 12/28/2018.
Plan of Correction: This was an oversight. The provider will print new sworn statements of affirmation and all caregivers will sign one.
Standard 8VAC20-800-290
Sharp kitchen utensils and other sharp objects shall be inaccessible to children.

There were sharp kitchen utensils to include knives and scissors in an unlocked drawer in the kitchen, where children have access.
Plan of Correction: The provider recently had new cabinets put in and forgot to put a new lock on the drawer with the sharp objects. All sharp utensils will be kept in accessible to children.
