Inspector Notes
An unannounced renewal inspection was conducted on 04/26/2022 from 9:25 am - 11:00 am. At the time of entrance there were three children in care with the provider. Two additional children arrived at 10:00am. The ages of the children counted for a total of 10 points.
Children were observed during free play and circle time. Records were reviewed for five children, one household member and the provider.
Violations were found in the areas of administration and physical equipment.
These violations are listed on the violation notice and were reviewed with the provider at the conclusion of the inspection.

1. The note from the physician of child 2 did not provide all of the required information for a physical exam.
Evidence:
1. The following safety hazards were observed in the backyard play area:
a. A tree root, measuring approximately 16 inches, was protruding from the ground. This is a trip hazard.
b. Two tricycle seats were missing areas of plastic on the seat. These missing areas have resulted in cracked plastic with sharp edges.
The tree root was cut and removed after the inspection.
Evidence:
1. The enrollment record for child 1 lacked the following required information:
a. The address is not listed for both emergency contacts;
b. Proof of the child's age and identity was not on file.
2. A second emergency contact is not on file for child 2.
3. Documentation of the annual review of the child's emergency contact information was not on file for child 3, child 4, and child 5.
The birth certificate has been requested from the parent of child 1.
