Inspector Notes

An unannounced monitoring inspection was conducted on 11/29/2022 from 11:10 am - 12:25 pm. At the time of entrance there were five children in care with the provider. The ages of children counted for a total of 10 points.
Children were observed during free play, clean up and decorating the Christmas Tree with the provider.
Records reviewed for three children and the provider.

Violations were found in the area of administration and are listed on the violation notice issued to the family day home.

Based on record review, the provider failed to ensure that children's records contain all of the required elements.

Evidence:
1. The enrollment record for child 1 and child 2 lack documentation of the name, address and phone number for a second emergency contact.
Plan of Correction: The provider stated that a second emergency contact will be obtained for both children today at pick-up.
Standard 8VAC20-800-90-C
Based on record review, the provider failed to obtain documentation of additional immunizations.

Evidence:
1. The most recent immunizations for child 1 were administered on 09/03/2020. The immunization record on file does not list all required immunizations needed for child over the age of two years.
Plan of Correction: The provider stated that child 1 had obtained additional immunizations and that the updated immunization record has been misfiled. If the current record can not be located, the parent of child 1 will be asked to provide a new copy of all current immunizations.
