Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 11/8/202. The on-site inspection began at 11:00 am and ended at 12:34 pm. The inspector reviewed compliance in the areas listed above. There were 22 children present and five staff. The inspector reviewed five children?s records
This inspection included: document review, tour of the facility, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence which includes staff and parent signatures or two staff signatures.
Child records #3 & 6 had injuries reports that did not have the parents signatures or two staff signatures.
Plan of Correction: Staff will ensure that the injury documents are signed as required by the standards.
