Inspector Notes

An unannounced monitoring inspection was conducted today from 10:00am-10:50am with the provider on site. There were 4 children in care (14 points) with 1 caregiver. A sample of 2 children's files and 2 caregiver/household files were reviewed. The children were engaged in the following: playing with toys. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and caregiver were positive. The physical space, attendance records, drill record and the Emergency Preparedness Plan were reviewed. There were no medications on site. Areas of non-compliance are identified in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver names; list caregivers by positions only.

Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Based on review, the provider did not obtain the parent's written acknowledgement of the
receipt of the information of the policy for administration of medication.
Evidence:
1. Child #1's file (start date 9/18/2023) did not have the parent's written acknowledgement of
the receipt of the information of the policy for administration of medication.
Plan of Correction: I will ask the parent to sign it today.
Standard 8VAC20-800-90-C
Based on review of children's records, the provider did not obtain documentation of additional immunizations for a child once every six months for children under the age of two years old.
Evidence:
1. Child #2's file (start date 5/08/2023) had an immunization record dated 5/12/2023 and needed documentation of additional immunizations.
Plan of Correction: I will ask the parent for it today.
