Inspector Notes
A monitoring inspection was conducted on July 6, 2022 from 11:10 A.M.-1:45 P.M. There were 43 children present, ranging in ages from 1 year 8 months to 12 years of age, with seven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and twelve staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

1. Staff #3's date of employment was 04/22/2022. The tuberculosis screening on file was dated 03/08/2022.
2. Staff verified the date of employment for Staff #3 and the date on the tuberculosis screening.
Evidence:
1. Staff #2's date of employment was 01/11/2022. The Virginia Health and Safety Preservice training was dated 06/05/2022.
2. Staff #11's date of employment was 01/05/2022. The Virginia Health and Safety Preservice training was dated 06/05/2022.
3. Staff verified the dates of employment for Staff #2 and Staff #11, and the dates on the training documents.
Evidence:
1. Staff #8 only completed 8 hours of annual training between January 2021 and January 2022.
2. Staff verified that Staff #8 only had 8 hours of annual training documented.
Evidence:
1. On the school age playground there was a rocking chair that was missing a slate in the seat portion of the chair.
2. On the preschool playground there was a two wheel bike that had the handle bar grips missing from the handle bars exposing the metal ends and the seat of the bike was torn and ripped in several areas; there was a radio flyer trike that had the handle bar grips missing and exposing the metal ends; and a dump truck that was rusted in several areas.
Evidence:
1. In Classroom #4 there was a bottle of sunscreen that expired on 6/30/2022.
2. Staff verified the expiration date on the bottle of sunscreen.
