Inspector Notes
A monitoring inspection was conducted on April 8, 2024 from 10:00 A.M.-1:15 P.M. There were 36 children present, ranging in ages from two to five years of age, with eight staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations
6Evidence:
1.Staff #3 indicated on a sworn disclosure statement dated 06/28/2023 that they had lived
outside of Virginia within the last five years.
2. The out-of-state criminal history finding in the record was dated 11/22/2023. Staff #3's first date of employment was 08/28/2023. The criminal history check was required to be completed prior to the first date of employment.
Evidence:
1. The medication policy did not include the following required component's; non-prescription medication be consistent with the manufacturer's instructions for age, duration and dosage; and it did not include methods to prevent use of outdated medication.
2. In the Pre-K Classroom downstairs there was an emergency medication, Albuterol Inhaler, that had expired on 03/2024. The medication had not been administered.

2. Administration verified that the medication was not in the original box and was missing the required information.
Evidence:
1. In the Pre-K classroom downstairs, there was an Epi-Pen, Children Benadryl, and Albuterol Inhaler that were not in a locked location.
2. In Classroom #2 upstairs there was an Epi-Pen in the first aid bag. The first aid bag was not in a locked location outside on the playground.
Evidence:
1. There was no documentation for an evacuation drill completed in March 2024.
2. Administration verified that there was not an evacuation drill completed in March 2024.
Evidence:
The documentation log for January 2023-December 2023 only had one shelter-in-place drill documented.
