An unannounced inspection was initiated on November 20, 2025 and completed on November 20, 2025 in response to a complaint received by the licensing office on November 13, 2025 relating to supervision. This inspection included interviews, observations, and document review. The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
