Inspector Notes

An unannounced inspection was initiated on August 27, 2024 and completed on September 20, 2024 in response to a complaint received by the licensing office on June 20, 2024 relating to allegations of staff qualifications and training. A renewal inspection was conducted on June 24, 2024 and reviewed aspects of the complaint. This inspection included interviews and document review. Additional interviews were conducted off site.

The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
