Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 11/25/2024 and completed on 11/26/2024. The on-site inspection began at 11:10 AM and ended at 12:30 PM. The inspector reviewed compliance in the areas listed above. There were 45 children present and12 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.

Child # 8 who has been in attendance for over two years did not have an updated immunization since the enrollment date of the child.
Plan of Correction: The director notified the parent that additional immunizations are required.
Standard 8VAC20-780-500-B
A nonabsorbent surface for diapering or changing shall be used.

There was a fabric covering on the changing pad in the infant room.
There was a tear in the changing pad in the toddler room.
Plan of Correction: During the inspection the director ordered additional changing pads.
