Inspector Notes
An unannounced renewal inspection was conducted today from approximately 11:30am-3:30pm. There were no children in the direct care and supervision of 1 caregiver. A sample of 3 children?s records, and 2 caregiver's records and 1 household member's record was reviewed. Zero new caregivers have been hired since the last inspection. Caregiver A stated she has to get fingerprints for household member A.
Evidence:
Household Member A did not have a sworn statement on file. Caregiver A said the household member would complete the form.
Evidence:
Caregiver A stated training hours weren't complete for 2021.
Evidence:
The first aid kit was missing 2 triangle bandages.
Evidence:
The provider stated that she drives the children to and from school, however there was no written permission in the file of the children being driven.
