Inspector Notes
An unannounced renewal inspection was conducted today from approximately 11:40am-12:50pm. There were 4 children in the direct care and supervision of 2 caregivers. A sample of 4 children?s records, and 2 caregiver's records were reviewed. The children were observed napping. Zero new caregivers have been hired since the last inspection. Violations
8Evidence:
1) Caregiver A stated she travels to pick up children while Caregiver B watches the children.
2) The times worked of Caregiver B was not recorded.
Evidence:
The tuberculosis screening of Household Member A expired 10/04/2023.
Evidence:
Caregivers A and B completed zero hours of annual training in 2022.
Evidence:
The toaster and blender located on the kitchen counter were plugged in and not being used.
Evidence:
Four electrical outlets located in the living area were not covered with protective covers.
Evidence:
The record of daily attendance was not kept from 10/02/2023 through 10/26/2023.
Evidence:
1) The record of Child A was missing the first day of attendance, the father's residence information, the father's employer information, the mother's work number, mother's personal phone number, liability insurance waiver, the address of Child A, and field trip permission.
2) The record of Child B was missing the father's work phone number, Child B's home address, the employer phone number of the mother, the mother's phone number, mother's home address, insurance policy number, proof of identification, field trip permission and liability form.
3) The record of Child C was missing the father's phone number, the father's employer phone number, the mother's employer phone number and the insurance policy number.
4) The record of Child D was missing their birthday, first day of attendance, Child D's address, the father's phone number, the father's work number, the employer of the father, the mother's work phone number, the phone number of 1 emergency contact, liability insurance waiver, field trip permission form, updated immunization, provisions of emergency preparedness, and complete proof of identity.
Evidence:
The first aid kit was missing scissors and 2 triangle bandages.
