Inspector Notes

An unannounced inspection was initiated on 6/6/25 and completed on 9/10/25 with local agencies in response to a complaint received by the licensing office on 5/30/25 relating to allegations of inadequate care of children. This inspection included document review, tour of the home and interviews. The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/17/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-130
Caregivers shall be of good character and reputation and be courteous and respectful to the children in care.

Caregiver #2 did not demonstrate good character and respect and was not courteous or respectful when initiating and engaging in repeated unwelcomed, inappropriate physical interactions with school-age Child #4. Child #4 reported the inappropriate physical interactions with Caregiver #2 occurred on a weekly basis for over 3 years until May 2025. When Child #4 would cry, Caregiver #2 told the child to be quiet and not to tell anyone because something bad would happen to both of them if discovered by others.
Plan of Correction: Will pay attention to children's behaviors to look for possible change in behavior
Standard 8VAC20-800-30-E
The provider shall ensure that the home's activities, services, and facilities are conducive to the welfare of children in care.

The frequent and recurring inappropriate interactions between Caregiver #2 and Child #4 went unnoticed for over 3 years by the provider.
Plan of Correction: Will look carefully into all children's behaviors and well-being.
Standard 8VAC20-800-580-A
In order to promote a child's physical, intellectual, emotional, and social well-being and growth, caregivers shall provide respect and personal privacy.

Over the course of at least 3 years, Caregiver #2 did not promote school-age Child #4's physical, emotional and social well-being and growth or respect the child's personal privacy when initiating and engaging in repeated, unwelcomed, inappropriate physical interactions with Child #4. Child #4 reported that the interactions with Caregiver #2 were forced, caused physical pain, emotional distress and fear. At times the child cried and tried to get away from Caregiver #2. .
Plan of Correction: Will look for trainings that enforce children's physical, intellectual, emotional, and social-well being and growth.
Standard 8VAC20-800-640-1
The following acts or threats thereof are forbidden; physical punishment including, but not limited to, roughly handling a child or forcing a child to assume an uncomfortable position.

While being supervised at the family day home over a period of at least 3 years, school-age Child #4 had frequent recurring incidents of unwelcomed, inappropriate interactions with Caregiver #2. Child #4 reported that the physical interactions with Caregiver #2 included being roughly handled and being forced to assume uncomfortable positions which caused physical pain, crying, emotional distress and fear. Another child in care observed and reported the inappropriate physical interactions between Caregiver #2 and Child #4 in May 2025.
Plan of Correction: Will be more cautious and make sure no acts or threats are being done to any children in care.
