Inspector Notes
An unannounced on-site focused monitoring inspection was conducted on 12/9/25, as the Department issued a Notice of Intent to revoke the license on 10/27/25 as a result of high-risk violations. The on-site inspection began at 10:13am and ended at 11:43am. The inspector reviewed compliance in the areas listed above. There was 1 child present and 2 caregivers; the point total was 2. The inspector reviewed 1 household member's record while on-site. The inspection included document review, tour of the home and interviews. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
