A complaint inspection was conducted in response to several complaints received by the Office of Childcare Health and Safety related to ratios, food service and physical plant. Violations related to this complaint are on the attached violation notice.

Based on observation, it was determined that the facility did not ensure that when children were in mixed aged groups, the ratio to the youngest child is applicable.
Evidence:
There was an ongoing mixed age group of children during the inspection that contained 19 children ages two through school age. Since the youngest child was two the required ratio of 1 staff for every eight children. There were only two staff present with this group. An additional staff member was required.
