Inspector Notes

A complaint inspection was conducted on February 7, 2024, in response to a complaint that was received by the department on January 17, 2024, regarding allegations in the care of children at the center. The inspector arrived at 10:20am and departed at approximately 12:15pm. There were 28 children present and 7 staff members. Staff were interviewed, documentation was reviewed, classroom observations were completed, and a tour of the facility was conducted. The evidence gathered during the investigation did support the allegations of non-compliance with standards or law with regards to procedures of emergencies. The complaint is determined to be valid. A violation was documented and is listed on the violation notice discussed with the director at the exit interview.

Based on record review and interview, it was determined that the center did not ensure that they maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the staff and parent signatures or two staff signatures.
Evidence:
1. In reviewing the incident report written for January 15, 2024, there were no signatures of the staff and parent signatures or two staff signatures.
2. Staff #1 confirmed that there were no staff and parent signatures or two staff signatures to the injury report written for January 15, 2024.
Plan of Correction: The center responded with the following: The center will have all staff members re-trained in writing injury reports during the next staff meeting. All classrooms will have a binder to access an injury report log and Director will audit all injury reports to ensure they are filled out appropriately.
