During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced inspection was initiated and completed on 4/28/25 in response to a complaint received by the licensing office on 4/4/25 and 4/30/25 relating to allegations of forbidden actions and special care provisions. The inspector reviewed three children?s records and three staff records on-site on 4/28/25. This inspection included document review, interviews, and observations. The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to staffing/supervision and administration and are documented on the violation notice issued to the program.

Standard 8VAC20-780-140-B
If the child has had a physical examination prior to attendance, it shall be within the time period prescribed: within 12 months prior to attendance for children two years of age through five years of age.
The physical examination for Child #1 was not within the required time period for when physical examinations are done prior to attendance.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care and protection.
On 3/28/25, Staff #1(program director/owner) was in charge of supervising a group of children during rest time when the staff member left the room for two-three minutes, leaving the children unsupervised. When staff returned, a child was found crying, after inappropriate interactions with another child in care.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.
On 3/28/25, Staff #1 (program director/owner) left resting children unattended in a classroom for approximately two to three minutes. The children were three years of age.

For children preschool age, during the designated rest period, the following rest period ratio is permitted (children of preschool age: one staff per 20 children).
During nap time on 3/28/25, eight children, three years of age, were left alone and unsupervised by staff. One staff is required for every 20 children. The classroom was out of ratio due to the absence of the staff member.
