Repeat Violation
The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.
The results of the most recent inspection were not posted, which is required.

The center must request repeat background checks every five years. A repeat central registry search for Staff #3 was not obtained until over a year after the background check had expired.

At least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis (TB) screening. TB documentation for Staff #2, Staff #3, and Staff #4 were expired on the day of the inspection. Staff #2 and Staff #4 were observed working during the inspection.

Repeat Violation
Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care. There was no documentation to show that Staff #2 and Staff #3 completed 16 hours of annual training in 2025, which is required.

After the first license, annual approval from the health department shall be provided. There was no documentation at the center to show that an environmental sanitation report was completed within the year.

Repeat Violation
Areas and equipment of the center, inside, shall be maintained in a safe condition. In a one year old classroom, a shelf was blocking the egress door, which is used for exit in case of an emergency.

Lockdown procedures shall be practiced at least annually. There was no documentation at the center to show that at least one lockdown drill was practiced in 2025, as required.

Repeat Violation
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain all the required information. The record for Child #1 did not contain all the required information.

All staff who work directly with children shall have within 90 days of the date of
employment current certification in cardiopulmonary resuscitation (CPR). The CPR certification for Staff #1 and Staff #5 show that the certification was not received within 90 days of employment, as required.

The first aid certification for Staff #1 and Staff #5 show that the certification was not received within 90 days of employment, as required.

Repeat Violation
Staff who work directly with children shall, complete the department's health and safety update course, annually. There was no documentation to show that Staff #4 completed the department's health and safety update course in 2025.
