Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on 02/24/2022 and concluded on 03/14/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 17 children present with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of three staff records and interview, the center did not ensure one staff member obtained a fingerprint based background check determination letter prior to employment.

Evidence: 1) The fingerprint determination letter in the record for Staff #1, hired on 02/24/2022, was dated 07/07/21. 2) During interview, a member of management reported Staff #1 was previously hired by the center in July 2021, but was terminated from employment in August 2021. Staff #1 was rehired on 02/24/2022, but new fingerprints were not completed prior to the first day of employment.
Plan of Correction: Moving forward, all rehires will complete new fingerprints prior to hire.
