Inspector Notes
A monitoring inspection was conducted on 02/06/2024 from approximately 10:03 a.m. ? 2:50 p.m. There were 34 children present ranging from age 3 to age 4 and seven staff supervising. Nine child records and five staff records were reviewed. The information gathered during the inspection determined violations with applicable standards or law.
Please sign and complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days.
Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

6Evidence:
The staff record for S3, employed 09/26/2023 documented S3 having lived out-of-state in the last five years. The record did not contain the results of the out-of-state criminal history name check. The criminal history check was conducted by Truth Finder, not the state?s criminal record agency.
Based on record review, the provider failed to ensure that staff shall complete the Virginia
Department of Education-sponsored orientation course within 90 days of employment.
Evidence:
The record for S4, employed 09/13/2023 contained a certificate of completion of the
Virginia Preservice Training dated 01/20/2024.
Evidence:
The table used by children in the 3-year-old room is missing trim molding exposing raw wood.
A can of wall paint was observed in the 4-year-old bathroom.
The plastic play items on the playground (kitchen and boat) had broken plastic areas that can snag children?s skin.
The protective plastic covering the chain on the climbing structure on the playground is dry rotted and can snag children?s skin.
Based on review of the center?s written record of children serious or minor injuries the center
failed to document required information.
Evidence:
The written records for serious or minor injuries for C6 (10/10/2023), C7 (12/07/2023), and C9 (09/19/2023) did not document any future action to prevent recurrence of the injury. The injury report for C8 (12/13/2023) did not document the time when parents were notified of the injuries.

Licensing inspector observed multiple lunch boxes and water bottles in classrooms that were not labeled with the child?s name or dated.
Evidence:
The center did not notify the Virginia Department of Education of a child injury for C9 that occurred on 09/19/2023. The center submitted the report on 02/07/2024.
