Inspector Notes
An unannounced, on-site monitoring inspection was completed on 01/15/2025. The on-site inspection began at 10:17 a.m. and ended at 3:10 p.m. The inspector reviewed compliance in the areas listed above. There were 40 children present and 8 staff. The inspector reviewed 5 children?s records and 6 staff records. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 02/21/2025. A POC submitted after this date will not appear on the public website.
Violations
5access by children. A bottle of antiacid was observed in the kitchen drawer that was not in a locked place. A tube of zinc oxide sunscreen was observed in the 4-year-old classroom (front classroom) that was not in a locked place.
The center shall maintain a written record of children?s serious and minor injuries that contains all the required information. The injury report dated 01/09/2025 for C6 did not contain the parent signature or a second staff signature. The injury report dated 10/16/2024 for C5 did not contain the time of parental notification. The injury report dated 10/17/2024 for C7 did not contain the actions to be taken to prevent reoccurrence.
When food is brought from home the food container must be clearly dated. Multiple thermos bottles brought from home were observed in the center older 4-year-old room were not dated.
