Inspector Notes

An unannounced, on-site renewal inspection was completed on 08/08/2025. The on-site inspection began at 10:06 a.m. and ended at 2:12 p.m. The inspector reviewed compliance in the areas listed above. There were 22 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/27/2025. A POC submitted after this date will not appear on the public website.

Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass sufficient area to include the child's trajectory in the event of a fall. Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus. The climbing tower, police car, spinner, and slides on the playground did not have a minimum of 6 inches resilient surfacing as required.
Plan of Correction: We have added new resilient surfacing under one outdoor play structure. We have purchased more rakes and shovels for the teachers to move the gravel back under play structures before coming inside daily.
