Inspector Notes

/An unannounced inspection was initiated on 4/17/25 and completed on 4/23/25 in response to a complaint received by the licensing office on 3/21/25 relating to allegations of supervision and behavior guidance. The inspector reviewed 2 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to staff records and qualifications and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/29/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-D
In each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present.
During the week of 4/14/25 through 4/18/25, the center's school age program was operational 5 full days. During this time, three staff members were responsible for the supervision and care of children. None of the three staff members met the qualifications of a program leader.
Plan of Correction: We have begun training for one of the staff to become lead qualified. She has begun training.
Standard 8VAC20-780-70
Staff records must include two written reference checks as part of the staff record.
1. Staff # 1's first reference check was obtained 3 days after the first date of employment.
2. A second written reference check was not obtained for Staff # 1 to this date.
Plan of Correction: Another reference has been added to the staff's file. For future reference check we will have it before staff first day.
