During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, focused inspection was conducted on 3/9/26 from 5:53pm to 6:00pm, via telephone. The inspection was conducted in response to the receipt of an online injury report for a child in care requiring outside medical attention. The inspector reviewed compliance in the areas listed above. The inspector reviewed 1 children?s record electronically on 3/3/26. This inspection included document review, interviews, and observations. Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 03/19/2026. A POC submitted after this date will not appear on the public website.
Violations
11. The terms of the license for the center state that there shall be no more than 29 children on the Preschool Playground at any one time.
2. On 2/25/26, a four year old child sustained an injury on the Preschool Playground, that required outside medical attention.
3. The injury report submitted to the department by the center stated, and Staff # 1 confirmed during a phone conversation that there 34 children on the playground with 4 staff members.
