Inspector Notes

An unannounced, on-site renewal inspection was initiated on 07/24/2024 and completed on 07/24/2024. The on-site inspection began at 9:30 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were 4 children present and 1 staff. The inspector reviewed 4 children?s records and 2 staff records on-site on 07/24/2024. This inspection included:

? document review of injury logs, policies and procedures, emergency drill logs, medication, authorization forms, 3 household records,
? tour of the facility, and
? interviews.

Information gathered during the inspection determined (1) non-compliance with an applicable standard, and the violation was documented on the violation notice issued to the program.

Standard 8VAC20-800-90-C
Based on review, 1 of 5 children records indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years.

Evidence -
1. On the date of inspection (07/24/2024), an immunization on file for Child #5 was dated - 09/15/2023.
2. On the date of inspection, Child #5 was under two years of age.
Plan of Correction: The parent of Child #5 was contacted on the date of inspection. The documentation will be updated by tomorrow.
