Inspector Notes

A Subsidy Health and Safety inspection was conducted on 08/21/2024 to review supplemental health and safety requirements. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and
safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.

Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on
08/29/2024. A POC submitted after this date will not appear on the public website.

Based on record review, the vendor did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence:
The last documented hard copy record of daily attendance was taken on August 8th 2024.
Plan of Correction: Daily attendance is now being completed daily.

Based on record review, all caregivers did not complete the 10 hour Virginia Preservice Training for Child Care Staff.

Evidence:
Caregiver #2, employed since October 2020, has not completed the required 10 hour VA Preservice training course.
Plan of Correction: 10 hours of training is being worked on by all staff.

Based on record review, all caregivers did not complete the annual health and safety update course.

Evidence:
Caregiver #1's last documented training of the health and safety course was taken in June 2019.
