Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, and documentation review..

A monitoring inspection was conducted on February 15, 2022. The provider was contacted by telephone and a virtual inspection was conducted. There were 6 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, one staff member did not have the results of a central registry check on file within 30 days of assuming job responsibilities.
Evidence:
Adult 1 did not have the results of a central registry check on file. The background check was submitted in the required time frame, but the results have not been received and documented follow up with the agency was not available for review on the date of inspection.
Plan of Correction: I did send it again for the second time and it was expedited. I will inform you ASAP.
Standard 8VAC20-800-170-A
Based on record review, the provider did not obtain, at the time of hire, a negative Tuberculosis screening form published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form.
Evidence:
Adult 1's record did not contain the results of a negative Tuberculosis screening.
Plan of Correction: I have it now.

Based on record review, one of two children's records did not contain all required information.
Evidence:
Child 1's record did not contain addresses for two persons to contact in the event that parents cannot be reached.
Plan of Correction: I already talked to the parent and got it done.
