During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on July 3, 2025 and completed on July 3, 2025. The on-site inspection began at 11:15 am and ended at 12:15 pm The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 caregivers; the point total was 17. The inspector reviewed 3 children?s records and 4 caregiver/household member(s) records [on-site] .
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 10, 2025. A POC submitted after this date will not appear on the public website.

Substitute providers' records shall also contain documentation of the time of arrivals and departures as required by 8VAC20-800-140 D.

On the date of inspection a there was no documentation of the substitute provider's hours of operating as a substitute provider.
Plan of Correction: I printed a time sheets for both of them and they started using it.
Standard 8VAC20-800-240-A
Areas and furnishings of the family day home, inside and outside, shall be maintained in a safe, and operable condition.

landscaping fabric was observed under the outdoor play structure posing a tripping hazard.
Plan of Correction: I bought mulch and changed it from all wood mulch to rubber mulch and it is safer now.

The family day home shall keep a written record of children in attendance each day.

The attendance for the family day home was not completed for the date of inspection.
Plan of Correction: I had a big training session with my assistants and they said they will never forget it.

Annually, the provider shall:
Review with the parent the emergency contact information to ensure the information is correct, and obtain the parent's signed acknowledgment of the review.

The annual review for Child 2 is overdue by 140 days.

The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care.

During the inspection no documented evacuation drill practices for 2025 could be located.
Plan of Correction: I have it in the wall but it was hidden so I put it on the wall and everyone can see.

The family day home shall obtain documentation of additional immunizations, for a child who is not exempt from the immunization requirements, every six months until the age of two.

Child 2's immunizations are overdue by eighteen months.

Child 3's immunizations are overdue by eleven months.
Plan of Correction: I talked to the parents. One of them brought it and the other one will ASAP.
