Inspector Notes

The Licensing Inspectors conducted a complaint inspection on November 1, 2022 in response to a complaint that was received in the Licensing office on October 28, 2022 related to the supervision of the children in care. Staff were interviewed, documentation was reviewed, and the entire facility was inspected and observed during the investigation. The evidence gathered during the investigation does not support the allegation, therefore the complaint is deemed to be not valid. During the course of the complaint investigations, there were concerns with staff records in which violations were cited. The violations were reviewed with the center's Director at the exit interview.

Based on record review and interview, it was determined that the center did not ensure that they obtain a copy of the results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
1. Staff #2 (date of hire 7/20/22) did not contain documentation of a completed child abuse and neglect registry check.
2. Staff #1 confirmed that there was no CPS registry check for Staff #2.
Plan of Correction: The center responded with the following: Staff #1 will request the required CPS Child registry checks and place the results to their record.

Based on record review and interview, it was determined that the center did not ensure that for staff hired after March 1, 1996, documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering.
Evidence:
1. Staff #2 (date of hire 7/20/22) did not contain two required reference checks to their record.
2. Staff #1, Director, confirmed there were no references to Staff #2's record.
Plan of Correction: The center responded with the following: Staff #1 will obtain references for Staff #2.
