Inspector Notes

A complaint inspection was conducted on August 9, 2023 and concluded on August 18, 2023. A complaint was received by the department on July 29, 2023 regarding allegations in the areas of the care and supervision of children. The inspector conducted a tour of the facility that included observations of the staff and program related to the allegation. Staff were interviewed, records and documentation were reviewed. Based on the information gathered during the investigation the complaint was found to be not valid. During the inspection a violation was found that was not related to the complaint. The violation was reviewed with the director and is listed on the violation notice.

Based on interviews and record review it was determined the center did not ensure that parents shall be provided at least semiannually in writing information on their child?s development, behavior, adjustment and needs.
Evidence:
1. The record for Child #1 did not include documentation that semiannual information was given to the parent.
2. The Director confirmed the center did not provide this information to the parent.
